Problem: What problem is this initiative trying to address?
1-Chronic diseases are epidemic worldwide in children and adults, including heart disease, diabetes, hypertension and obesity
2-Chronic diseases now account for the majority of health care costs worldwide (86% in the U.S.).
3-Chronic diseases thus threaten health insurance programs worldwide. "Our so-called health system ... is not sustainable. We cannot afford it." (Nestle retired CEO and Chairman Peter Brabeck).
4-Water is scarce worldwide.
Solution Summary: What is the proposed solution? What do you see as its most promising aspects for creating shared value?
1-Do what most people think is impossible: Reverse & halt heart and many other chronic diseases in children and adults by treating the cause with Plant Based Nutrition (PBN), which also drives prevention. The science and clinical practice are robust but little known to doctors, and the public. See attachments & videos for detail.
2-Implement in two ways.
(1) Focus on U.S. 110 million who receive their health benefits from employers & unions. Advantages are (a) innovation needed in benefit design and how to pay providers is much easier, as the C-suite is decision maker & no government or lobbyist approval required, (b) the long-term treatment needs of patients aligns with long-term financial interests of self-insured payers, unlike the short-term financial interests of insurers facing 1-year rating & fee-for-service providers.
(2) Trigger both innovations by government & insurance company health programs worldwide
3-Switching from animal food to PBN saves enormous amounts of water
Impact: What is the impact of the work to date? Specify both the social and the environmental impact of your work
1-Dr. Esselstyn, our partner and a recognized authority worldwide on reversing and preventing heart disease with plant based cuisine has treated about 1,000 people with about 90% adherence for years.
President Bill Clinton video (above) and our website demonstrates this solution works and saves substantial money by avoiding costly procedures like coronary bypasses and stents.
2-There are 500-1000 doctors who similarly use plant based cuisine to reverse and halt a number of chronic diseases. See the American College of Lifestyle Medicine website for more information on successes, the science, who they are and global actions.
3-Switching from animal food to PBN can save enormous amounts of water. One cow alone requires over 2.5 million gallons of water over its life.
4-Patient Value, LLC is a new company that focuses on making Plant Based Nutrition a new option for people with heart and certain chronic diseases in self-insured employer and union health plans in the U.S.
Financial sustainability plan: How is this initiative financially supported? How will you ensure its financial sustainability long-term?
a-95% personal investment by principal, 5% from clients. Substantial sweat equity by 3 principals (bios in PV Intro attachment).
b-Our revenue model that focuses on the self-insured 110 million sponsors has three parts.
(1) A PBN reversal program for people with, say, heart disease that choose the program (various program types)
(2) Recurring outcome payment, say yearly, for reversal and maintenance of a person's chronic disease, based on payer savings (in development). Two attached papers detail.
(3) A network of PBN providers nationwide is being developed, like Preferred Provider Organizations were created in the 1980s
c-Food companies seeking 5 new revenue opportunities, insurers, other entrants & governments are potential strategic partners or consulting clients

There are many retail PBN providers with 1-7 day programs & a Medicare 3 month Ornish program.
There are also many wellness & prevention programs.
No one else in the U.S. focuses on
1-and has the expertise with the self-insured 110 million
2-reversal first (not prevention), &
3-solving the fundamental flaw in how we pay providers & insurers short-term for the chronically ill's long-term needs, & is developing new recurring payments for the chronically ill's lifetime
Founding story: Share a story about the "Aha!" moment that sparked the beginning of this initiative.
Dr. Esselstyn presented his reversal of heart disease with PBN and the underlying science at a community center attended by Chuck Weller. Chuck sees it as the biggest breakthrough in health care treatment and cost reduction he has seen in 40+ years.
Chuck is a lawyer who has represented self-insured employers like General Motors for decades, but had never heard it was possible to reverse heart and other diseases that account for about 80% of employer and other private and public payer costs.
As a legal matter, self-insured employers have the unique advantages of being able to decide today to offer new benefit options like PBN and how to pay for them, nationwide, without government, insurer or lobbyist approval.
Neither of us had heard of the other's areas of experience and expertise before.
